Physical Therapy Checklist: What to Bring to Your First Appointment

Medical Now’s Physical Therapy Checklist

Before arriving to your first physical therapy appointment, make sure to bring all of the documents related to your treatment. Here’s a Physical Therapy Checklist including all of the information you need for your appointment.

Identification 

Bring your ID and if you have a physical therapy referral (provided to you by your doctor). If your insurance is covering the cost of physical therapy, bring your insurance card.

Hospital Records

If you were in an accident and were in the hospital or received medical treatment please bring operative reports, X-ray and/or MRI results that you have pertaining to your injury.

Police Report

If you were in a car accident please bring your police report.

Claim Number

If you are covered by Workers’ Compensation, bring your claim number and your case manager’s contact information. For more information on filing a Worker’s Compensation claim, click here to visit NY State’s website.

Vehicle’s Insurance

For car accident or NYS No Fault: if you are covered by auto insurance or an attorney lien, make sure you bring this information.
